​Participating in the contest is not required to be published. Once we've received this form from you, we will be publishing you either way because we think you're awesome! The contest just provides even more prizes and opportunities that we can offer our Inspiring Teens, 

The overall winner with the most votes will be on the cover and possibly win a scholarship! 
* Teens must be between ages 13-19 and live in the continental USA to win a scholarship. Teens cannot win more than $1000 in a year's time.    
* Each vote is $5.00 and goes towards scholarships & prizes, the magazine production, & our teen programs.

    The next questions starting with IQ1 -IQ5. are your actual mini-interview questions up until the point you receive your personal interview in Level 3. Please answer thoughtfully ( and in complete sentences) as they will go up on the blog when you reach that point of the contest if you reach Level 2 but not 3. They will be included in your personal interview questions (about 10 more questions pertaining to these answers) when you reach Level 3.
